*Merchandise order must be placed on a single invoice. Combining of invoices to reach total is not allowed. Subtotal of merchandise order must be equal, or exceed prize total prior to sales tax, shipping or web/fax discount if applicable. Orders must be placed between October 1st and December 31st, 2008.

The promotion period is 90 days therefore we are unable to include invoices prior to or after promotional dates. All qualifying invoices must be redeemed no later than January 15th. Requests after that date will not be honored. Multiple orders accepted throughout the quarter. Please allow 4-6 weeks for delivery of free good, after the date of claim.

Merchandise that is not applied to this total includes: items found in the equipment section of the Goetze Dental 2008-2009 Catalog with the exception of Ultrasonic Inserts. Bag styles and colors subject to manufacturer's availability.
